What to Expect When Sending a Referral

Contact Referral

We then contact who you’ve referred, and gather information such as their location, what services they need, and the hours/days they need support. 

Internal Documentation

Next, we enter intake documentation into our system and our scheduling team coordinates with nurses to get admission scheduled.

In-Home Assessment

Our nurse conducts an in-home and safety assessment and completes intake.
