Application & nurse assessment

We help with the CFI application (Form 800 and notarized Form 770). A registered nurse then completes a Medical Eligibility Assessment and a care plan is created.

Choose your caregiver & get paid

Your loved one chooses a trusted family member or friend. We handle onboarding, paid training, payroll, and ongoing support — with weekly pay every Friday.

Who Qualifies

CFI eligibility at a glance

Eligibility applies to the person receiving care. If they qualify for CFI, they choose who provides their care.

The person receiving care

  • Lives in New Hampshire
  • Is 65 or older, or 18–64 with a Social Security disability designation
  • Needs help with 2+ daily activities — nursing-facility level of care
  • Income under $2,982/month (individual, 2026) and assets within NH Medicaid limits
  • Has NH Medicaid, or is applying for it

Who can be the paid caregiver

  • A family member, friend, or neighbor — and in some cases a spouse
  • 18 or older and legally eligible to work in the U.S.
  • Able to pass a background check
  • No nursing certification required — we provide training
  • Committed to following your loved one’s care plan

Two Ways to Get Care

Family caregiver first — with professional care if you need it

Most families start by getting a loved one paid through CFI. If no one is available, our caregivers can step in.

Become a paid family caregiver

Already helping a parent, relative, or friend with daily care? Through CFI, you may be able to get paid for the care you already provide.

Got questions about the CFI program?

Choices for Independence (CFI) is New Hampshire’s Medicaid home and community-based services waiver. It helps seniors and adults with disabilities who need nursing-facility level of care stay safely at home — including with care from a family member or friend.

Yes. Under CFI’s self-directed option, your loved one can choose a family member, adult child, friend, or neighbor as their paid caregiver — and in some cases a spouse. Medicaid pays the caregiver; the person receiving care pays nothing for covered services.

No nursing certification is required for family or friend caregivers. Caregivers must be 18 or older, legally eligible to work in the U.S., and pass a background check. We provide paid training so you feel confident from day one.

For 2026, the individual income limit is $2,982 per month, and the asset limit for an individual is $2,500. If your loved one is slightly over these limits, planning options may help — we can point you in the right direction.

The application is Form 800 plus a notarized Form 770, filed online through NH EASY, at a DHHS office, or through ServiceLink. A registered nurse then completes a Medical Eligibility Assessment. We guide you through every step.

CFI enrollment is limited, so a waitlist may apply. Starting the application early — and getting it right the first time — helps avoid delays.

Yes. CFI services can be provided in your loved one’s home, a family member’s home, an adult family care home, or an assisted living or residential care setting. CFI doesn’t cover room and board in those settings.
