Veterans Deserve Care That Meets Them Where They Are

Veterans in New Hampshire may qualify for in-home support through the VA Community Care Network (CCN), which connects eligible individuals with Home and Community-Based Services (HCBS).

These services are designed to help veterans remain safely at home while receiving the care and support they need.

How Guardian Home Care of NH Can Help

Guardian Home Care of NH works with the VA Community Care Network (CCN) to connect eligible veterans with in-home care services.

We help:

  • Coordinate with VA care teams and authorizations
  • Match veterans with qualified caregivers
  • Deliver consistent, personalized in-home support
  • Ensure services align with approved care plans

Our goal is to make the process simple, clear, and supportive—so veterans and their families can focus on care, not paperwork.

Do Veterans Have To Pay For These Services?

In many cases, services are covered through VA programs when eligibility requirements are met.

That means veterans can receive care at home without paying out of pocket, depending on their approved benefits.

If you’re unsure whether you qualify, our team can help you understand your options.

How It Works?

Authorization & Care Coordination

The VA reviews eligibility and approves the type of services and number of hours. Once authorized, our team coordinates with the VA to confirm the care plan, schedule, and expectations.

In-Home Assessment

Our nurse provides an in-home assessment to evaluate home safety and create a personalized care plan for the veteran’s needs.

Client Stories

“Anne is hands down the most wonderful aide I’ve ever had. She takes care of everything I need–laundry, cleaning, and more. She’s always there for me, even in the middle of the night. I really appreciate all her hard work, and I just love when she comes by. Anne is the sweetest, and I want everyone to know how much she means to me”.
