Desiree Young, MBA
Director of Operations
With over 14 years of experience in the healthcare industry, Desiree Young brings a wealth of expertise to her role as Director of Operations at Guardian Home Care of NH. In addition to leading New Hampshire operations, she also serves as the Chief Operating Officer for Guardian Home Care USA, overseeing national expansion and strategic development.

Desiree’s career began as a Home Health Aide, where her passion for serving others laid the foundation for her leadership in healthcare. She has since earned her credentials as a Licensed Practical Nurse, a bachelor’s degree in Healthcare Administration, and an MBA in Healthcare Management. At Guardian Home Care of NH, she works closely with her team to streamline operations, enhance service quality, and ensure clients and their families receive the best possible care. Her dedication to compliance and excellence was exemplified when she successfully led her New Hampshire team through a deficiency-free state survey. She remains committed to empowering caregivers and fostering a culture of compassionate, high-quality home care.

Sheila Kimani
Business Development & Office Director
As the Business Development and Office Director at Guardian Home Care of NH, Sheila Kimani brings extensive experience and a deep commitment to compassionate care. Her background as a Case Manager for the Choices for Independence (CFI) program provided her with invaluable insight into the needs of clients, family caregivers, and case management partners, equipping her with a well-rounded perspective on home care services.

Prior to her work as a case manager, Sheila served as a Personal Care Assistant (PCA) and Home Health Aide (HHA), giving her firsthand experience in the impact of quality care within the home setting. This foundation allows her to approach her leadership role with both empathy and expertise.
In her current position, Sheila plays a vital role in coordinating client care with case managers to ensure seamless services and support for individuals in need. She also assists in overseeing the daily operations of Guardian Home Care of NH, working closely with the Chief Operating Officer to uphold excellence in service delivery and maintain the organization’s strong track record of deficiency-free state survey results.

Cara Lombardi, RN
Clinical Director
Cara Lombardi, RN, is the Clinical Director at Guardian Home Care of NH and a third-generation nurse with over 12 years of experience.

She began her career in acute care before discovering her passion for community and home health in 2016. She is dedicated to helping individuals thrive in their home environments, believing deeply in the power of compassionate, person-centered care. Cara’s approach is grounded in the simple yet profound idea that at the end of the day, we’re all just people helping people. Alysha Gabriel
Admissions & Marketing Coordinator
As the Admissions and Marketing Coordinator for Guardian Home Care of NH, Alysha Gabriel brings a deep passion for helping others and a solid background in healthcare administration to support individuals and families during key life transitions.

With over eight years of hands-on experience as a Licensed Nursing Assistant (LNA), she has a firsthand understanding of the challenges and rewards of direct patient care—an insight that informs her approach to service coordination and community engagement. Alysha approaches admissions with empathy, professionalism, and a strong commitment to quality care. Whether guiding families through the intake process or building outreach strategies to reach more people in need, she is dedicated to making a meaningful difference in the lives of those Guardian Home Care of NH serves.

Anna Guerette
Recruiting Lead
As the Recruiting Lead at Guardian Home Care, Anna Guerette plays a key role in connecting compassionate individuals with meaningful careers in caregiving.

Since entering the healthcare field in 2021, she has witnessed the profound impact of care grounded in empathy, trust, and dignity. She is proud to help build a team that reflects these values and provides essential support to individuals and families. Anna is currently completing her degree in Health Information Management. Outside of work, she enjoys a quiet, creative life with her husband and their two cats, who add plenty of love—and a touch of mischief—to their home. She finds joy in comforting and creative activities like reading, photography, and knitting, and cherishes time spent with loved ones sharing meals, making memories, and celebrating the simple moments that make life meaningful.

Donna Marcotte
Lead LNA and Client Support Coordinator
Donna Marcotte brings years of hands-on experience and dedication to her role as Lead LNA and Client Support Coordinator at Guardian Home Care of NH.

Donna began her career in home care as a Personal Care Service Provider, working in the field for over a decade before earning her Licensed Nursing Assistant (LNA) certification in 2015. In her current role, she provides guidance to caregivers, supports client care planning, and assists with scheduling to help ensure consistent, high-quality service. Donna is known for her reliability, compassion, and strong commitment to both clients and coworkers. Outside of work, she enjoys relaxing at the beach, attending concerts, and playing bingo. She is also a proud YaYa and cherishes time spent with her grandchildren and family.

Patrick Trent-McGranaghan
Community Liaison
Patrick Trent-McGranaghan brings over a decade of experience in the Human Services field to his role as Community Liaison at Guardian Home Care of NH.

A lifelong New Hampshire resident, Patrick has a deep-rooted passion for building a better world and a strong belief that everyone deserves dignity, security, and respect. Having worked in both direct service and case management roles, Patrick understands the benefits available to New Hampshire residents—and the barriers they often face in accessing the care and services they are entitled to. In his work, he strives to make a meaningful difference in the lives of individuals and families by helping them stay connected to their communities and loved ones for as long as possible. Outside of work, Patrick enjoys tinkering with computers, hiking and traveling with his wife and partner, storytelling and gaming with lifelong friends, and participating in local community theater.

Tatiana Ballesteros
Community Liaison
Tatiana Ballesteros serves as a Community Liaison at Guardian Home Care of NH. Originally from Bogotá, Colombia, Tatiana is a trained attorney and native Spanish speaker who is also fluent in English.

Her legal career and volunteer work have centered on supporting individuals with compassion, integrity, and a strong sense of social responsibility. Tatiana is passionate about making a difference in people’s lives and brings warmth, empathy, and dedication to every interaction. She is proud to be part of the Guardian team, where her values and background align with the mission to support individuals and families with dignity and care.
