This is a Medicaid program that helps NH residents who are Medicaid eligible or active to receive care at home, whether the caregiver is someone we screened and trained or a family member, friend, or neighbor.

Do You Qualify For CFI?
If you're Medicaid eligible or active, and have a physical or mental ailment that poses a risk to your safety or prevents you from accomplishing daily living tasks, you very well may qualify for the CFI program. How We Can Help
Our caregivers are trained to offer a wide range of personal care services under these 3 categories.

Personal Care At Home
Qualifying for CFI means you're approved to receive home care for a certain numbers of hours each week. The personal care services can range from light housekeeping, meal prep, feeding, grooming, transfer, walking, transferring, shopping and errands, companionship, and custodial nursing.

Trusted Caregiver
Depending on your situation, you may choose to have personal care delivered by one of our screened and trained caregivers or elect someone you know and trust, like a family member, friend, or neighbor.

Pay and Benefits For Family Caregivers
If you choose to be cared for by someone you know (like a family member, friend, or neighbor), we will work closely with your CFI case manager and make sure your caregiver receives wages and benefits for the care they provide you.
