Whether your client is eligible for the Frail Elderly (FE), Physical Disability (PD), or Brain Injury (BI) Waiver, exploring grants under the Older Americans Act or Senior Care Act, or seeking in-home support with private pay, we’ll work with you to find the right care path, quickly and professionally.

We connect directly with the referred client or family, gather details about their situation, and begin the intake process.

Eligibility Review & Documentation

Whether the client has a waiver, may qualify, or could be eligible for grant-funded care, we coordinate with MCOs, assessors, or partners to complete documentation.

In-Home Assessment

Our nurse conducts an in-home and safety assessment and completes intake.

Wrap Up Intake

We complete the plan of care and wrap up intake.

Start of Services

We match the client with a qualified caregiver—or onboard the caregiver they’ve chosen in a self-directed model, then services can start.
