Taylor B. Hampton
With a decade of experience in healthcare and administration, along with a strong background in volunteer service, Taylor brings both operational expertise and a servant’s heart to the Guardian leadership team. Her vision is centered on building a culture that empowers caregivers and supports families. She is dedicated to providing reliable, compassionate care for every client while creating an exceptional workplace where caregivers feel valued, supported, and equipped to grow. As Guardian Home Care of IN expands its reach, Taylor is focused on building a strong foundation for serving communities throughout the state and creating a lasting, positive impact. Beyond her professional life, Taylor is a proud wife and mother of two and remains dedicated to helping others in every facet of her life.

Tessa Noland
Tessa brings six years of experience as an Indiana Medicaid Waiver Coordinator, providing coordination to waiver members across Indiana. Her extensive knowledge of Indiana Medicaid and waiver programs gives her a valuable understanding of the care process, the role of coordinators, and the challenges individuals and families may face when seeking support at home. In her role at Guardian Home Care of Indiana, Tessa works closely with clients, families, caregivers, schedulers, administrators, and care coordinators to address concerns, maintain clear communication, and help ensure each client’s needs are being met. She also serves as a direct point of contact for coordinators managing mutual cases. Tessa earned her bachelor’s degree in Psychology from Indiana University and previously spent three years working in the mental health field with both children and adults. This experience helps her consider more than an individual’s physical care needs and bring a broader understanding to client and family advocacy. Her goal is to become a trusted resource for clients, families, and coordinators seeking knowledgeable guidance on Indiana Medicaid waiver programs and quality home care options. Outside of work, Tessa is a mother of three and enjoys creating stained glass art.

Cara Lombardi, RN
She began her career in acute care before discovering her passion for community and home health in 2016. She is dedicated to helping individuals thrive in their home environments, believing deeply in the power of compassionate, person-centered care.
Cara’s approach is grounded in the simple yet profound idea that at the end of the day, we’re all just people helping people. She is also a proud mom of three and dog-mom to a goldendoodle named Pepper.

Patrick Trent-McGranaghan
A lifelong New Hampshire resident, Patrick has a deep-rooted passion for building a better world and a strong belief that everyone deserves dignity, security, and respect. Having worked in both direct service and case management roles, Patrick understands the benefits available to New Hampshire residents—and the barriers they often face in accessing care. In his role, he supports individuals and families during the intake process while also building strong community connections to ensure people can access the services they need. Outside of work, Patrick enjoys tinkering with computers, hiking and traveling with his wife and partner, storytelling and gaming with lifelong friends, and participating in local community theater.
