Three simple steps to trusted care — and a paycheck
From the ALTCS assessment to your first shift, our team is with you at every step.
Get assessed
An ALTCS assessment determines whether your loved one qualifies and how many hours of care they can receive. Not enrolled yet? We’ll guide you through the application.
Choose a caregiver
Your loved one chooses someone they trust — a spouse, son, daughter, relative, or friend. We handle onboarding, background checks, and paperwork.
They get care — you get paid
Once approved, you’re set up as your loved one’s paid caregiver, with Guardian Home Care of AZ supporting you with scheduling, payroll, and ongoing guidance.
ALTCS eligibility at a glance
Eligibility is based on the person receiving care. If they qualify for ALTCS, they can choose who provides their care.
The person receiving care
- Lives in Arizona
- Is 65 or older, or has a qualifying disability
- Needs help with daily activities — nursing-home level of care
- Has ALTCS, or meets its income and asset limits
- Can direct their own care, or has a representative who can
Who can be the paid caregiver
- A spouse, adult child, relative, or trusted friend
- A legal guardian, in some cases
- 18 or older and able to provide the care needed
- Able to pass a background check
- Doesn’t have to live in the same home in most cases
Medicare alone isn’t enough — ALTCS is an Arizona Medicaid program. Not sure if your loved one qualifies? Check eligibility or call 602-529-5508.

Got questions about ALTCS and paid caregiving?
ALTCS (the Arizona Long Term Care System) is Arizona’s Medicaid program for long-term care. It helps seniors and people with disabilities who need nursing-home level of care receive that care at home instead — including care from a family member or friend.
Yes. Under ALTCS, a spouse can be approved as a paid caregiver. Adult children, other relatives, friends, and in some cases legal guardians can also be paid caregivers.
In most cases, no — a family caregiver doesn’t have to live in the same home as the person they care for. We’ll confirm the requirements for your situation.
Arizona residents who are 65 or older or have a qualifying disability, need nursing-home level of care, and meet ALTCS income and asset limits. Medicare alone isn’t enough.
You apply through AHCCCS (Arizona’s Medicaid agency), complete an eligibility interview, and have a medical assessment. If approved, you choose a health plan that manages your care. We guide you through each step.
We can help you understand eligibility and guide you through the application. If you need care sooner, we also offer private pay home care.
