Contact Client or Family
We connect directly with the referred client or family, gather details about their situation, and begin the intake process.
Eligibility Review & Documentation
If applicable, we help review ALTCS status, discuss eligibility considerations, coordinate with case managers, and gather information needed to move forward with services.
In-Home Assessment
We complete an in-home assessment and intake process to better understand the individual’s needs, preferences, safety considerations, and care goals.
Wrap Up Intake
We finalize service agreements, care planning, scheduling preferences, and caregiver matching.
Start of Services
We match the client with a qualified caregiver or help onboard an eligible family caregiver through the Family Care Attendant program when applicable. Once services are authorized and onboarding is complete, care can begin.
